The Health and Social Health Services have to face the increasing demand of new and suitable answers to the needs of a growing population of patients getting older, suffering from chronic degenerative diseases, in advanced or terminal stadium, in clinical conditions of extreme fragility and severe distress.
In such a context, the overall vision of the patient, the attention to the symptoms control and to the quality of life, the appropriateness and proportionality of the assistance plan, the teamwork and the coordination of the network are key elements for the development of a project focused on excellence.
Italia Longeva, within its objectives of promoting quality of active life of the elderly population, in collaboration with interRAI Italy promoted an event to present the state of the art of the application in Italy of the multidimensional assessment tools for the management of people in complex chronic conditions more popular in the world: the tools of interRAI Suite.
